assess patient needs when booking an appointment

Besides our staff, we share this information with ACC (when required) in order to claim ACC payment.

Providing some information is optional. If you choose not to enter name, date of birth, and other relevant information when booking an appointment, we'll be unable to provide osteopathic treatment.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
